• Phone Number: If you provided one when creating an account, or if you opt in to SMS meal logging, in which case we also verify it

• Caregiver Information: If you use Well Plated as a caregiver, information you provide about yourself and your relationship to the person you support

Health and Nutrition Information

Depending on how you use Well Plated, we may collect:

• Photos of meals you submit
• Descriptions of food and meals
• Meal logs and related information
• Weight, height, and other health information you or your care team record
• Check-in responses and related safety information
• Health information you choose to share with Well Plated through connected health services
• Information generated from the information you submit, such as estimated meal and nutrition information

Communications

The Service includes in-app messaging that allows you to communicate with connected providers and caregivers and to interact with Well Plated features such as the Meal Logger. Messages and other communications exchanged through the Service are collected and stored to provide these features, support communication and care coordination, and provide relevant guidance.

If you choose to use SMS meal logging, we collect the text messages and images you send to our service number to log your meals. These messages may also be used to support meal logging, identify support needs, and provide relevant guidance or recommendations.

Usage Information

We automatically collect certain information about your device and use of the Service, including device type, operating system, and app usage patterns.

Website Analytics

We use privacy-focused website analytics to understand how people find and use our website, including campaign information in the links we share. This may include the site that referred you, campaign labels, pages viewed, and interactions with the site.

How We Use Your Information

We use the information we collect to:

• Provide, maintain, and improve our Service
• Process and organize meal logs based on photos and descriptions
• Send SMS messages related to meal logging (if you opt-in)
• Communicate with you about your account and the Service
• Analyze usage patterns to improve user experience
• Understand how people find our website, including which of our outreach brings them there
• Protect against unauthorized access and ensure security
• Comply with legal obligations

How We Share Your Information

We do not sell your personal information. We may share your information in the following circumstances:

Service Providers

We work with third-party service providers who assist us in operating the Service:

• Twilio: Processes SMS messages for our text-based meal logging feature
• OpenAI: Provides AI-assisted meal analysis and nutrition insights
• Supabase: Provides secure database and authentication services
• Vercel: Hosts our web services and provides analytics for our website

These providers are contractually obligated to protect your information and may only use it to provide services to us.

Healthcare Providers

When you connect with a provider through Well Plated, they can access your Well Plated record to support your care, including information recorded before you connected. This may include information about you and your treatment; meal logs, photos, and Meal Logger conversations; check-ins, responses, and safety information; weight and height information; information and notes recorded by your care team; and Apple Health information you've chosen to share with Well Plated, including weight, steps, and active energy.

Your private messages with other providers are not shared. You can unlink a provider at any time. After you unlink, that provider retains access to records from the period when you were connected but does not receive new information.

Caregivers

When you connect with a caregiver, they can see:

• patterns in your meal logging over time, but not meal details or estimated calories
• patterns in your weight over time, but not your weight or other exact numbers
• patterns in your movement over time, if you share Apple Health data with Well Plated, but not exact numbers
• your preferred name, which is the name displayed to connected caregivers and providers

Legal Requirements

We may disclose your information if required by law, legal process, or government request, or when necessary to protect the rights, property, or safety of Well Plated, our users, or others.

Data Security

Well Plated follows HIPAA privacy and security practices to protect your information.

We implement appropriate technical and organizational measures to protect your information against unauthorized access, alteration, disclosure, or destruction. These measures include:

• Encryption of data in transit and at rest
• Secure authentication and access controls
• Regular security assessments
• Limited employee access to personal data

However, no method of transmission over the Internet or electronic storage is 100% secure. We cannot guarantee absolute security.

Data Retention

We retain your information for as long as your account is active or as needed to provide you with the Service. You may request deletion of your account and associated data at any time, and we will process deletion requests within 30 days. Some information may be retained as required by law or for legitimate operational and legal purposes.

The Service is not intended for children under 13 years of age. We do not knowingly collect personal information from children under 13. If we learn we have collected such information, we will delete it promptly.
